Delivery receipt template (proof of delivery)
Updated · Reviewed by Israel Margulies, founder and CEO of High Point GPS
A delivery receipt for the moment goods reach the customer: what was ordered, what was delivered and in what condition, any shortage or damage, and the receiver's name and signature with the date and time. It's your proof of delivery.
A signed delivery receipt settles most delivery disputes before they start. It shows the customer received the goods, how many and in what condition, and when. Write any shortage or damage on the receipt at the door, with the receiver, not in a call a week later.
Use it for deliveries from your own trucks, equipment drop-offs and pickups. For freight moving with a carrier, the bill of lading travels with the shipment and the receiver signs that too.

Fields: Delivery / order no. · Customer · Delivery address · Date · Time · Driver · Truck no.
Log columns: Item · Description · Qty ordered · Qty delivered · Condition
- Exceptions. Write any shortage, damage or refused items here, before the receiver signs.
- Received. The receiver confirms the items above were delivered in the quantity and condition shown, except as noted.
Fill it in the right way
- Fill in the order number, customer, address and the items before the truck leaves.
- At the stop, count the items with the receiver and check their condition.
- Write the quantity delivered and the condition of each item.
- Note any shortage, damage or refused item under Exceptions.
- Have the receiver print their name and sign, and sign as the driver.
- Take a photo of the delivered goods and leave a copy with the customer.
Tips
- Get a printed name with the signature; a scribble alone doesn't say who signed.
- A photo of the goods at the drop point backs up the receipt.
- Use the same number on the order, the receipt and the invoice.
- Record the exact time; it answers "when did it arrive?" and shows how your ETAs hold up.
Do it in FleetZap
- FleetZap Dispatch plans deliveries and pickups, optimizes the route and sends the customer an ETA.
- Drivers record proof of delivery on their phone at the stop.
- Each delivery stays on the plan with its truck and driver.

A document the receiver signs to confirm goods arrived, listing what was delivered, the quantity and condition, with the date, time and any exceptions.

A signed delivery receipt is one form of proof of delivery. Photos, GPS location and an electronic signature on a phone are others.
What should I do if goods arrive damaged?
Note the damage on the receipt before signing, with photos, and tell the sender right away.
How is a delivery receipt different from a bill of lading?
The bill of lading covers a shipment with a carrier from pickup to delivery; the delivery receipt records what the receiver got at the stop.
