Equipment sign-out sheet (tool checkout log)
Updated · Reviewed by Israel Margulies, founder and CEO of High Point GPS
A running log for the tool crib or equipment room: every time someone takes a tool or a machine, they write the date, the item and its tag, their name, the job, and its condition, then sign it back in. You always know who has what.
Tools and small equipment walk away when nobody writes down who has them. A sign-out sheet kept at the crib or the yard gate turns "has anyone seen the core drill?" into a name and a job site, and it shows which items come back damaged and which never come back.
Use this sheet for many items over time. For a single machine going out to one person or customer with a detailed condition check, use the equipment checkout form instead.
Equipment sign-out sheet
Fields: Location / tool crib · Sheet no. · Month
Log columns: Date out · Item and tag no. · Taken by · Job / site · Condition out · Date in · Condition in · Initials
Fill it in the right way
- Put a tag or number on every tool and machine you lend out.
- Keep the sheet where items leave: the crib window, the yard gate, the van.
- Fill in a line each time an item goes out: date, item and tag, name, job and condition.
- When it comes back, write the date, its condition and initial the line.
- Check open lines every week and chase what's overdue.
- Send anything that comes back damaged for repair before it goes out again.
Tips
- Tag numbers beat descriptions: there are always three "grinders".
- Make the person taking the item write their own name; it's what makes them bring it back.
- Look for patterns: the same item coming back damaged, or the same crew losing tools.
- Weekly checks of open lines find a missing item while someone still remembers where it is.
Do it in FleetZap
- Every machine and tool can carry a QR label; scanning it opens its record and history on a phone.
- Problems reported by scanning the label open a repair work order for the shop.
- Each item's location, documents and repair history stay on its record.

Frequently asked questions
What is an equipment sign-out sheet?
A log of who takes which tool or piece of equipment, when, for which job, and when it comes back, with its condition both ways.
What's the difference between a sign-out sheet and a checkout form?
A sign-out sheet is a running log for many items and people. A checkout form records one item going to one person or customer, with a detailed condition check and signatures.
How do I stop tools going missing?
Tag every item, make people sign out what they take, check open lines weekly and follow up quickly. Knowing who had it last solves most cases.
Should damaged tools go back on the shelf?
No. Tag them out and send them for repair, so the next person doesn't take a broken tool to a job.
