Safety meeting sign-in sheet

Updated · Reviewed by Israel Margulies, founder and CEO of High Point GPS

A sign-in sheet for safety meetings and toolbox talks: what was covered, who led it, and the name and signature of everyone who attended.

A safety meeting only counts if you can show who was there and what was covered. A signed sheet with the date and topic is that record for training files, client audits and insurers.

Keep the sheet with the talk's notes or handout, so the topic and the attendance stay together.

Safety meeting sign-in

Fields: Date · Time · Topic · Presenter · Location / job site · Length of meeting

Log columns: Name · Company · Job / trade · Signature

Fill it in the right way

  1. Fill in the date, topic, presenter and location before the meeting.
  2. Pass the sheet around or have each person sign as they arrive.
  3. Have the presenter sign at the end.
  4. File the sheet with the talk's notes, and follow up with anyone who missed it.

Tips

  • Print names clearly next to signatures; signatures alone are hard to read later.
  • Keep a separate sheet for each meeting, not one long list.
  • Record the topic precisely, for example 'excavator swing radius', not just 'safety'.
  • Store sheets by date so you can show a year of meetings quickly.

Do it in FleetZap

  • Machine issues raised in a safety meeting can be reported by scanning the machine's QR label, which opens a work order.
  • Inspection procedures can include signature steps, so checks carry the inspector's name.
  • FleetZap records checks and repairs; it doesn't track training records.
